We are open and we are accepting new patients. You can easily book, reschedule and cancel your appointments using our booking site.

NEW PATIENTS

Digital patient intake forms will be emailed to you once your new patient appointment has been scheduled. Please fill out and SIGN the forms PRIOR to your first appointment. We know your time is valuable. We want to maximize the amount of time you spend with the physiotherapist during your first visit rather than spend time filling out paperwork.

Please wear comfortable clothes. All assessments are one on one with your physiotherapist. During that hour, your physiotherapist will assess your condition and discuss patient oriented goals that will direct your treatment.

We offer direct billing for most insurance companies. Your insurance plan might require that you get a doctor’s referral for physical therapy, which you need to bring on your first appointment. You also have the option of emailing it to us along with any additional reports like MRIs, X-Rays, etc. Also, please take into consideration we cannot guarantee direct billing for each and every patient. It all depends on the type of benefit plan you have with your insurance company.

Hussein Khakee BA. Kin, MSc. PT, IMS Certified

Hussein completed his Bachelors degree in Kinesiology from York University (2011) followed by his Masters degree in Physiotherapy from the University of Toronto (2014). Moreover, he has completed post graduate education in McKenzie, MDT rehabilitation for the spine, dry needling, vestibular rehabilitation, as well as various orthopedic approaches to care. He has undertaken roles in clinical education, practice and management, developing lectures and algorithms to aid in the diagnosis and treatment of various conditions. He espouses evidence based practices and prides himself on being accountable and accessible to the clients he serves, optimizing their reintegration to function post impairment or injury.

Grace Yamamoto

Grace joins us after graduating from Queen’s University with a Master of Science in Physical Therapy. Prior to this, she received a BSc in Biomedical Science from the University of Ottawa, where she conducted recently published research in the field of pelvic floor physiotherapy.

Grace’s passion for the field of rehabilitation stems from personal experiences with physiotherapy as a young athlete and various volunteer and work opportunities. In addition to orthopedic clinics, she has experience working in hospital settings such as medical and surgical acute care units and in-patient rehabilitation.

Grace believes that educating and empowering patients to be an active participant in their rehabilitation journey is the key to a successful recovery. She values providing an evidence- based approach that is tailored to meet each individuals unique needs. Grace works collaboratively as part of the Brant Lakeshore Physiotherapy team to re-establish functional strength, range of motion and help individuals return to full and pain free living.

Outside of work Grace enjoys hiking, cooking, playing volleyball and spending time with family and friends.
